Une session internationale sur la surveillance du SARS-CoV-2 dans les eaux usées proposée par la section  Virologie de la SFM à l’ECCVID !

“Wastewater and COVID-19: a tool for disease and SARS-CoV-2 dynamic surveillance”

Le SARS-CoV-2 est éliminé dans les selles des personnes infectées (50% présentent des troubles digestifs) avec des titres viraux allant de 10E4 à 10E9 unités de génome par millilitre. Sa viabilité dans les selles reste un sujet controversé.  Le virus peut cependant être retrouvé et quantifié par RT PCR dans les eaux usées et les stations d’épuration. La surveillance des eaux usées peut-elle permettre de suivre l’évolution de l’épidémie ? Voire de prédire les clusters ? Quel est le risque infectieux associé ? Trois orateurs ont présenté leur expérience.

1) L’expérience hollandaise de la surveillance hebdomadaire de 300 stations d’épurations et effluents d’hôpitaux prélevés à partir du début de l’épidémie a été présentée par Ana Marta de Roda Husman (Ministère de la Santé et de l’Environnement, Hollande). Le virus est détecté dans les eaux usées dès les premières semaines de l’épidémie, à partir des cas asymptomatiques ou pauci-symptomatiques, puis suit l’évolution du nombre de cas sur l’ensemble du territoire, diminue et disparaît avec la fin de la première phase épidémique. Cet outil de surveillance peu coûteux s’intègre dans la surveillance plus large des autres pathogènes et des résistances aux antibiotiques, dans le cadre du concept « One Health ». Il est désormais en place en Hollande en complément du dépistage (MOH COVID-19 Dashboard open source).

2) Qu’en est-il de la transmission par les évacuations des eaux usées dans les immeubles collectifs et les environnements urbains ?
Aaron Packman, directeur du Northwestern Wastewater Center, USA, expose les facteurs de risque déjà suspectés pour le SARS-CoV-1, urbanisations à forte densité de population, faible niveau sanitaire exposant au risque d’aérosolisation à partir des évacuations et des toilettes, aggravés par les fortes pluies et les inondations. Au contraire, l’ensoleillement, la chaleur, les procédés de traitement par oxydation des eaux d’épuration, inactivent le virus. Rendant peu probable le risque lié à l’épandage de boues d’épuration issues d’effluents traités. Reste l’inconnue sur la survie du virus dans les milieux solides non traités.

3) L’expérience française du réseau Obépine était présentée par Laurent Moulin, Eau de Paris.
Devant la difficulté de prédire et de suivre la dynamique de l’infection par le SARS-CoV-2 en population, le réseau Obépine a été mis en place pour suivre la détection de SARS-CoV-2 dans les eaux usées en France.  A dater de juillet 2020, la surveillance de 50 stations a été mise en place, et 150 stations seront surveillées à partir de septembre, après organisation d’un contrôle de qualité national et proposition de protocoles communs pour harmoniser les résultats de quantification au niveau national. Les premiers résultats de la surveillance (mars-septembre) confirment son intérêt pour suivre la dynamique du virus lors d’un pic épidémique ou, sur des zones géographiques plus réduites, pour repérer précocement les foyers épidémiques à partir de personnes asymptomatiques, du fait de la très bonne sensibilité des techniques (10E3 unités de génome par litre) et du suivi quantitatif des charges virales.
