La 34ème édition de l’ECCMID a eu lieu du 27 au 30 avril à Barcelone. Le temps n’était pas au rendez-vous mais les jeunes microbes, si !

Résumé de la présentation
Une épidémie d’entérobactéries productrices de carbapénémase liée à la dissémination d’un clone de Klebsiella pneumoniae est apparue à l’hôpital Bichat en 2016 et se poursuit aujourd’hui. Ce clone est porteur d’un plasmide de résistance incF, pNDM_EPI1. Celui-ci est très stable au cours du temps avec l’apparition de seulement deux variants plasmidiques (PVA et PVB), PVB ayant acquis un groupe de gènes de résistance au mercure.

“Decoding plasmids in diagnostic and infection control”, un symposium présenté par Alessandra Carattoli qui a décrypté les mécanismes de transfert horizontal de gènes de résistance via les plasmides et comment les utiliser pour résoudre des questions épidémiques. Elle a surtout pu répondre à notre question “comment déterminer si deux plasmides apparemment proches sont identiques ?”.

Résumé de la présentation
Nous nous sommes demandé quel était le risque de résistance du CMV au létermovir en prophylaxie primaire et secondaire dans une population de greffés de cellules souches hématopoïétiques. Lors d’une prophylaxie par letermovir, il y a des blips de charge virale CMV et c’est ce que nous avons étudié, vérifiant si tous les blips étaient associés à la l’apparition d’une mutation de résistance au létermovir.

”When to say yes: selecting organ donors with known infections”, présentée par WOOLLEY Ann et MULARONI Alessandra. Présentation d’études sur la survie post-transplantation en fonction du type de donneur d’organe, lié au risque infectieux viral, bactérien ou non infectieux.

La spectrométrie de masse MALDI-ToF permet d’identifier facilement les bactéries, mais qu’en est-il des champignons ? Les systèmes industriels comme ceux de bioMérieux permettent aujourd’hui d’identifier efficacement les pathogènes fongiques les plus communs ! Mais pour les pathogènes plus rares, c’est la base MSI-2, une base non commerciale disponible en ligne, qui reste la plus performante.

Résumé de la présentation
Deux gènes de résistance au céfidérocol ont été identifiés par métagénomique fonctionnelle en utilisant Escherichia coli comme bactérie hôte. Ces gènes mènent à une augmentation significative de la CMI (jusqu’à 267 fois la CMI associée à la souche hôte sensible).
