L’Europe connaît depuis juin 2023 une recrudescence importante de la coqueluche après une circulation très limitée pendant la pandémie COVID-19. En France, c’est au début de l’année 2024 que le nombre de cas rapportés par le réseau RENACOQ a très fortement augmenté justifiant un appel à vigilance de Santé publique France (SpF) en avril 2024 et une confirmation du démarrage d’un nouveau cycle épidémique en juin 2024. SpF confirme en juin 2024 cette très forte augmentation des cas confirmés par PCR [Au 26 juin 2024, les données des 3 labos (Cerba, Eurofins-Biomnis) rapportent un total provisoire de 14 866 PCR positives sur un total de 67 161 tests réalisés pour l’année 2024 (données provisoires), soit un taux de positivité (TP) provisoire de 22,1% versus un TP de 3,7% pour l’année 2023]. Cette augmentation du nombre de test PCR coqueluche a vite été responsable de tensions d’approvisionnement des laboratoires de biologie médicale (LABM) par les fournisseurs de kits. La même rupture de stock est observée pour les milieux de culture spécifiques commercialisés (Regan-Lowe ou Bordet-Gengou).

La Société Française de Microbiologie (SFM) en collaboration avec la Société de Pathologie Infectieuse de langue Française (SPILF), le Groupe de pathologie Infectieuse Pédiatrique (GPIP), le Centre national de Référence de la coqueluche et autres bordetelloses (CNR), a participé au sein de la Commission Spécialisée Maladies Infectieuses Maladies Emergentes (CsMIME) du Haut Conseil de Santé Publique (HCSP) à la rédaction de l’avis relatif à la transmission de la coqueluche aux personnes à risque de forme grave du 30 juillet 2024. En accord avec cet avis consensuel, la SFM émet des recommandations spécifiques au diagnostic microbiologique pour guider les LABM dans l’utilisation raisonnée des tests PCR coqueluche.

Nous reprenons d’abord les recommandations éditées dans le REMIC 7e édition, paru en 2022 (ouvrage de référence édité par la SFM) :

  • Le diagnostic microbiologique de la coqueluche repose sur la PCR ou la culture à partir d’un prélèvement naso-pharyngé par aspiration naso-pharyngée (ASNA) ou par écouvillonnage naso-pharyngé profond (comme pour le diagnostic des viroses respiratoires).
  • Les sérologies n’ont pas leur place dans le diagnostic (et ne sont pas prises en charge par l’assurance maladie).
  • Les PCR spécifiques sont à privilégier (les performances des tests syndromiques par PCR multiplexées étant incomplètement évalués pour le diagnostic d’une coqueluche).
  • Des cultures méritent aussi d’être réalisées à visée sentinelle, notamment pour le suivi de l’antibiorésistance émergente aux macrolides, selon les recommandations du CNR (https://www.pasteur.fr/fr/sante-publique/CNR/les-cnr/coqueluche-et-autres-bordetelloses).

Les indications du diagnostic microbiologique varient en fonction de l’âge, du statut vaccinal et des symptômes.

Dans le contexte de la flambée épidémique, des situations de tension en réactifs ou en personnels sont rapportées ayant conduit la SFM à proposer une stratégie pragmatique que l’avis du HCSP a pris en compte dans ses recommandations afin de favoriser le diagnostic et le traitement précoce des cas pour rompre les chaînes de transmission.

Évoqué par la clinique, le diagnostic de coqueluche doit être étayé soit épidémiologiquement (nouveau cas dans le contexte de cas groupés d’un cluster avéré), soit microbiologiquement.

Selon les recommandations de la SFM, une PCR spécifique sur prélèvement naso-pharyngé doit être demandée dans les situations suivantes :

  • Nouveau-nés et nourrissons < 6 mois, ou > 6 mois non ou incomplètement vaccinés, avec toux quinteuse ou associée à des apnées ;
  • Enfants, adolescents et adultes vaccinés, avec toux > 7 jours sans cause évidente, y compris en cas de rappel de moins de 5 ans (possible perte de la protection vaccinale).

La culture sur milieux spécifiques est moins sensible que la PCR, mais elle permet de suivre l’échappement vaccinal (comme la perte d’expression d’un antigène vaccinal) et d’identifier la résistance aux macrolides.

La SFM préconise, en contexte de recrudescence des cas, et devant une PCR positive de réaliser si possible une culture à partir du même prélèvement et en cas de positivité de celle-ci d’adresser la souche au CNR. Sans passer par l’étape de culture, il est possible d’envoyer directement au CNR l’échantillon positif en PCR si celui-ci provient d’un nourrisson atteint de forme grave (en réanimation) ou si la charge bactérienne est évaluée élevée par la PCR (https://www.pasteur.fr/fr/sante-publique/CNR/les-cnr/coqueluche-et-autres-bordetelloses).

Les tests diagnostiques ne sont pas indiqués dans les cas suivants :

  • Toux > 3 semaines (excrétion nulle ; tester plutôt un éventuel cas secondaire récent) ;
  • Contact restant asymptomatique.

Dans le contexte de la flambée épidémique et en situation de tension en réactifs ou en personnels, il est possible de s’en tenir à la stratégie pragmatique suivante :

  • Ne pas tester un cas cliniquement suspect s’il est contact d’un cas confirmé microbiologiquement ;
  • Lors de la survenue de cas groupés dans une collectivité [par ex. hôpital, EHPAD (établissement d’hébergement pour personnes âgées dépendantes) …], ne tester que les trois premiers cas et considérer les autres personnes symptomatiques comme des cas.

 Ces recommandations, élaborées sur la base des connaissances disponibles à la date de publication de cet avis (20 août 2024), peuvent évoluer en fonction de l’actualisation des connaissances et des données épidémiologiques.
