La période du 18 au 24 novembre, comme chaque année, marque un temps fort avec deux événements pour rythmer cette semaine : la Journée européenne sur la sensibilisation aux antibiotiques, portée par l’European Center for Disease prevention and Control (ECDC) et la Semaine mondiale de sensibilisation à la résistance aux antimicrobiens, orchestrée par la Quadripartite (Organisation Mondiale de la Santé – OMS-, Organisation pour l’alimentation et l’agriculture – FAO -, Organisation mondiale de la santé animale – OMSA, Programme des Nations unies pour l’environnement – PNUE) autour du thème “Ensemble, prévenons la résistance aux antimicrobiens”. Des éléments de communication sont notamment disponibles dans les langues officielles des organisations.
A l’occasion de cette semaine, les ministères de la Santé, de l’Agriculture, de la Recherche et de la Transition écologique unissent leurs efforts pour présenter une nouvelle feuille de route interministérielle à horizon 10 ans, intitulée “Prévention et réduction de l’antibiorésistance, lutte contre la résistance aux antimicrobiens”. Cette initiative s’inscrit dans une démarche globale “Une seule santé”, consciente des enjeux stratégiques pour la santé publique en France, en Europe et dans le monde. Cette feuille de route interministérielle fait suite à une précédente feuille de route dédiée depuis 2016. Dans la poursuite des efforts engagés depuis 2016, les cinq axes identifiés dans cette feuille de route se déclinent en 17 objectifs stratégiques dont la traduction opérationnelle fera l’objet d’une concertation dans les prochaines semaines. Cette feuille de route complète également les approches sectorielles portées dans la Stratégie nationale 2022-2025 de prévention des infections et de l’antibiorésistance en santé humaine, et dans le plan Ecoantibio 3 2023-2028 dans le champ de la médecine vétérinaire, publié le 18 novembre 2023.
La Direction de la recherche, des études, de l’évaluation et des statistiques (DREES) a publié le 20 novembre un outil de datavisualisation des proxy-indicateurs permettant d’approcher la pertinence de prescription d’antibiotiques chez les médecins généralistes. Douze proxy-indicateurs ont été couplés à des cibles à atteindre (optimale, acceptable, à atteindre). Chaque indicateur est calculé en niveau et en part de prescripteurs ayant atteint la cible. Les proxy-indicateurs sont présentés sous forme de graphiques, cartes et tableaux téléchargeables. Ces indicateurs sont regroupés en 3 thèmes : favoriser les antibiotiques de première intention, prescrire à bon escient, éviter les prescriptions d’anti-inflammatoires en cas d’infections.
Le RéPias a relayé de nombreuses publications sur la thématique. Le site met en ligne et répertorie des outils de communication et ressources disponibles en ligne sur la thématique, avec par exemple des vidéos « Antibiostories », produites par MATIS à destination du grand public.
Santé publique France publie trois documents en ce mois de novembre :
- Le 13 novembre, Santé publique France a publié ses données de consommation d’antibiotiques en secteur de ville pour l’année 2022. Le rapport mis en ligne relève notamment une augmentation des prescriptions et de la consommation d’antibiotiques en 2022, pour la deuxième année consécutive. Plus de 800 prescriptions d’antibiotiques pour 1 000 habitants ont été réalisées au cours de l’année (hors hospitalisation), soit une augmentation de 16,6 % par rapport à 2021. Le rapport met en lumière une hausse de 41,8 % des prescriptions et de la consommation d’antibiotiques en 2022 par rapport à 2021 chez les enfants âgés de 5 à 14 ans.
- Santé publique France a également publié son Bulletin épidémiologique hebdomadaire sur l’Antibiorésistance en 2023 : de la surveillance en santé humaine vers une approche « une seule santé ».
- Enfin, Santé publique France et ses partenaires publient leur synthèse « Prévention de la résistance aux antibiotiques : une démarche “Une seule santé” ». Ce document présente les principaux résultats de surveillance de la consommation d’antibiotiques et de la résistance des bactéries aux antibiotiques pour l’année 2022, dans les domaines de la santé humaine, animale et de domaine de l’environnement. Il expose également les actions mises en œuvre pour la prévention, le contrôle des infections et le bon usage des antibiotiques.
Au niveau européen, seuls neuf États membres ont réussi à atteindre ou à dépasser l’objectif fixé à 65 % de consommation d’antibiotiques du groupe “Access” selon la classification AWaRe de l’OMS, plaçant l’UE à 59,8 %. Ces antibiotiques sont préconisés en première ligne pour les infections courantes, minimisant ainsi l’émergence de la résistance aux antimicrobiens. L’ECDC souligne la nécessité d’interventions plus robustes dans divers domaines tels que l’utilisation prudente des antibiotiques, la prévention des infections, la recherche, la surveillance et la mise en œuvre de plans d’action nationaux “Une seule santé”.
Du 20 au 24 novembre se déroule également la semaine de la sécurité des patients (SSP). Depuis 2009, cet événement vise à mobiliser tous les acteurs concernés par ce thème. Cette campagne de sensibilisation offre l’opportunité de renforcer la collaboration entre les professionnels de la santé, les patients, leurs proches et la société dans son ensemble. Le ministère met également à disposition une boite à outils.
Enfin, le projet Happy Patient, initié par la Commission Européenne en janvier 2021, se terminera en décembre 2023. Il vise à réduire l’impact de la résistance bactérienne découlant de l’utilisation inappropriée des antibiotiques dans le cadre des infections communautaires en renforçant l’impact des recommandations européennes en faveur de l’usage prudent des antibiotiques en santé humaine. Cette approche se concentre sur les interactions entre patients et professionnels de santé de 4 secteurs : Médecine Générale, Pharmacies d’officine, Soins d’Urgence et Soins non Programmés, EHPADs. Dans le cadre de ce projet, de nombreux outils de communication ont été développés à destination des patients et des professionnels de santé (cartes simples à destination des patients pour expliquer l’antibiorésistance, modèle d’ordonnance de non-prescription pour les médecins, fiche dédiée à la délivrance des antibiotiques par le pharmacien). L’ensemble des outils est disponible au téléchargement.
