La ministre du Travail, de la Santé et des Solidarités, Catherine Vautrin a dévoilé la nouvelle feuille de route 2024-2027 de lutte contre les pénuries de médicaments. Cette feuille de route 2024-2027 fait suite à la première feuille de route mise en œuvre de 2019 à 2022.
La nouvelle feuille de route pluriannuelle repose sur quatre axes principaux :
- Axe 1 : Détection du signal et plan d’action gradué face aux tensions d’approvisionnement et aux pénuries. L’axe 1 mentionne le fait de favoriser les ordonnances de non prescription pour améliorer l’usage des antibiotiques, ainsi que la mise en place de mesures d’épargne exceptionnelles en cas de pénuries d’antibiotiques (recours systématique aux TROD pour les antibiotiques en pénurie, dispensation à l’unité obligatoire).
- Axe 2 : Nouvelles actions de santé publique pour améliorer l’accès aux médicaments. Cet axe 2 comprend notamment un point dédié pour le bon usage des médicaments essentiels, dont les antibiotiques, avec une accentuation prévu des actions de communication et de sensibilisation (grand public et professionnels), un renforcement des leviers d’incitation et l’expérimentation de nouveaux dispositifs.
- Axe 3 : Nouvelles actions économiques pour améliorer la disponibilité des médicaments
- Axe 4 : Transparence de la chaîne d’approvisionnement : l’information jusqu’au patient
Le rapport pluri-institutionnel JIACRA 4 a récemment été publié. Rédigé dans une perspective Une seule santé, en collaboration entre l’Autorité européenne de sécurité des aliments (EFSA), le Centre européen de prévention et de contrôle des maladies (ECDC) et l’Agence européenne des médicaments (EMA), il montre que les pays ayant diminué leur consommation d’antibiotiques chez les animaux et les humains ont observé une baisse des bactéries résistantes aux antibiotiques.
Il souligne la nécessité :
- De poursuivre les efforts pour lutter contre la résistance aux antimicrobiens (RAM) à l’échelle nationale, européenne et mondiale, tant dans les secteurs de la santé humaine que dans ceux de l’élevage ;
- D’établir une surveillance coordonnée de la consommation d’antimicrobiens et de la résistance aux antimicrobiens dans les domaines humain et animal ;
- De mener des études ciblées pour améliorer la compréhension de la propagation de la RAM.
Santé Publique France a récemment rendu publics les résultats de la surveillance nationale des consommations de produits hydro-alcooliques (PHA) dans les établissements d’hébergement pour personnes âgées dépendantes (Ehpad), avec les données 2021-2022. Après une augmentation significative entre 2019 et 2020 des consommations de PHA dans les Ehpad français, dans le contexte de la pandémie de Covid-19, la tendance à la baisse observée en 2021 se confirme en 2022. En 2021, 11,7 % des Ehpad ont enregistré une consommation de ≥ 4 friction hydro-alcoolique (FHA ) par journée de présence d’un résident, contre 6,7 % en 2022.
Malgré tous les biais potentiels, les résultats de cette surveillance soulignent la nécessité d’accroître les efforts de promotion de l’hygiène des mains via l’utilisation des FHA dans les Ehpad.
